NEW PATIENTS
Family Foot Center would like to
make your trip to our office as timely and as efficient
as possible. Therefore, we give you the option of
filling out your paperwork before you even step foot
in our office. Our doctors want to provide you with
the best case possible. It is extremely important
that you supply our office with accurate, up-to-date
information about your medical/surgical history as
well as proper billing information. Please bring a
complete list of all medications you are currently
taking (include name, dosage & prescription directions),
most recent insurance card as well as the insured's
name, date of birth and employer, an accurate billing
address, telephone number and other relevant information
and copies of office notes and x-rays pertaining to
your current medical condition, if applicable. Complete
